Ibutilide- Class-III Antiarrythmic drug and its therapeutic uses

DESCRIPTION
Ibutilide is an antiarrhythmic. It works by slowing the conduction of the irregular heartbeat throughout the heart, which allows an opportunity for the regular heart rhythm to take over.


PHARMACOKINETICS
1. Absorption is Rapid after intravenous injection.
2. Volume of distribution is 11 L/kg.
3. Metabolism is hepatic.
4. Half life is 6 hours.
5. In healthy male volunteers, about 82% of a 0.01 mg/kg dose of [14C] ibutilide fumarate was                excreted in the urine (about 7% of the dose as unchanged ibutilide) and the remainder (about              19%) was recovered in the feces.


THERAPEUTIC USES
It is used to treat atrial fibrillation or atrial flutter.


SIDE EFFECTS
Arrhythmia
Headache
Low blood pressure
Nausea
Palpitations (sensation that heart is pounding or racing)
Increased blood pressure
Bradycardia (slow heart rate)
Tachycardia (fast heart rate)


DRUG-DRUG INTERACTIONS
1. Ibutilide + Citalopram = Increase in the QTc-prolonging activities of Ibutilide.
2. Ibutilide + Propafenone = Increase in the arrhythmogenic activities of Ibutilide.
3. Ibutilide + Mifepristone = Increase in the QTc-prolonging activities of Ibutilide.
4. Ibutilide + Leuprolide = Increase in the QTc-prolonging activities of Ibutilide.
